Once your athlete is registered, please print out and sign all registration documents in the AYF PLAYER PACK. You will need to turn all required forms to a scheduled paperwork drop off date for processing or by the first day of practice.
If this is your athletes first year with the Raiders, we will need an original birth certificate with a raised seal for CMYFCC verification.
Additionally, you will need to provide a copy of your athletes End of Year report card for the 2021-2022 school year and a physician signed medical clearance form before your athlete can practice. You must have the 'AYF Medical Clearance' Form from the official AYF Packet signed by your athletes Doctor. We cannot accept the separate 'Resume Medical Clearance' form as it is only used in the event of in season injury prior to returning to play.

We understand that situations come up and conflicts may arise with other activities. We are committed to processing a refund for the registration of your athlete if our program does not work for your family, regardless of reason, within the first three weeks.
Refunds will be issued upon written request to email@example.com at any point prior to the start of the season.
No refunds will be issued once teams are in play.
No refunds will be issued on or after August 20, 2022 for the Fall season.
